Services |
Skilled Nursing
- Registered Nurses experienced in home health with backgrounds
in acute care.
- Areas of Expertise include: geriatric, pulmonary, psychiatric,
ostomy/wound care, intravenous therapy, diabetes education,
and external therapy.
Physical Therapy
- Provide skilled therapy techniques
- Teach home exercise programs
- Endurance training post hospitalization
Occupational Therapy
- Instruction for self care and safety in the home.
- Teach Muscle re-education and skills used for daily
living.
Speech Therapy
- Identify and Assess communication cognition and swallowing
problems.
- Teach exercises to improve verbal and non-verbal skills,
Increase swallowing and/or cognition abilities.
Home Health Aides
- Certified, experienced care in the home which may include
bathing, dressing, and hygiene.
Social Services
- Assist in location resources for finances, food, transportation
and medication.
- Assist in obtaining alternative placement and respite
care.
- Offer counseling for health-related problems.

| To Qualify
for Medical Coverage |
If a Patient's care is to be covered by Medicare the patient must:
- Be under the care of a physician who will direct the home care plan
- Be confined to the home by illness or injury
- Need skilled nursing, physical, occupation, or speech therapy on an intermittent basis

for Private Insurance Coverage |
Most insurance companies will cover home health care visits that are cost-effective in helping to prevent hospitalization
